Change Your Name
Requests to change a name must be made in writing and mailed to Pioneer Investment Management Shareholder Services, Inc.,

To change a name on your account due to marriage, divorce or any similar reason, you need to send us:
Original letter of instruction signed by the owner with a Signature Validation Program Stamp.

Correcting a Tax Identification Number
- To change a tax identification number entirely, you must request the change in writing, sign your letter and mail it to Pioneer.
By Mail
Send a completed Form W-9 (certifying your tax identification number) to Pioneer Investment Management Shareholder Services, Inc., Box 55014, Boston, MA 02205-5014. Be sure to include your 14-digit Pioneer account number, social security number and phone number. To request a Form W-9 contact your investment professional, or call us at 1-800-225-6292.
